CORI Form
(Criminal Offender Record Information) In 2002, the Massachusetts legislature passed a law requiring all schools, public and private, to conduct criminal background checks on current and prospective employees, volunteers, school transportation providers, and others who may interact with children. This law is for the safety of children. This process is completely confidential. The Falmouth public schools require that a background check be performed once a year. ALL Volunteers In Public Schools (VIPS) MUST fill out a CORI form each year. Each institution is required to do their own forms, they are not allowed to share information. So even if you have filled out one through the Boy Scouts, Girl Scouts, your Religious Organization or anywhere else you volunteer, you still have to fill one out for VIPS as well. Keep in mind that CORI forms do take a few weeks to process, so if you plan on volunteering in the classroom, going on a field trip, or volunteering for a PTO function, you should fill one out now rather than waiting to the last minute. For CORI Form click here (pdf format) (note you will be leaving PTO website -form hosted by WHOI.edu) You must bring your CORI form to the main office at Teaticket along with a form of picture identification. One of Teaticket's secretaries will verify the form and submit it to the VIPS office for processing Our Building Coordinators are Ruth Pearce & Donna Randleman.
